ALOA 2026 Optometrist of the Year  Award Nomination Form

This is an annual award presented to a worthy Alabama Doctor of Optometry in recognition of distinguished service to the Alabama Optometric Association (ALOA), to the profession of optometry, on behalf of the visual welfare of the public, and/or to their community.

(Nominee’s eligibility should not be restricted to service rendered during the year the award is to be presented.) 

GUIDELINES FOR SELECTION

1. Nominations can be made by any member of the Alabama Optometric Association. (It is recommended that societies sponsor qualified individuals.)
2. Nominees shall have been active or educator class members of the Alabama Optometric Association for at least six (6) consecutive years.
3. The current President and President-Elect of the Association and previous year’s recipient of the award will not be eligible for this year’s award.
4. All nominations must be accompanied by supporting information and data to assist the Awards Committee in its consideration of the nominations for the award.
5. Nominations MUST BE SUBMITTED BY AUGUST 14, 2026, in order to be considered.
6. The decision of the Awards Committee is final.
